ABSTRACT
Introdução: As incisões cervicais sempre foram motivo de preocupação de diversos autores quanto ao aspecto funcional. Os pacientes, por sua vez, questionam por uma cicatriz cosmeticamente aceitável, fato que é de suma importância para o bem estar psicológico dos que se submetem a cervicotomias. As incisões mais anatômicas para a região cervical são as incisões cervicais transversas únicas, pois proporcionam uma boa exposição cirúrgica das estruturas anatômicas em todos os níveis de linfonodos e respeitam as linhas naturais de tensão da pele. Objetivos: Avaliar as incisões cervicais transversas únicas, rigorosamente colocadas na prega cervical superior (PCS) ou sulco cervical superior (SCS), realizadas em cirurgias de cabeça e pescoço para tumores malignos, observando às dificuldades técnicas, complicações, cura, funcionalidade e por último a estética. Métodos: Foram estudados 20 pacientes submetidos a cervicotomias na PCS ou SCS para esvaziamentos cervicais radicais clássicos (seis casos), modificados (quatro) e supraomoioideos (10). As abordagens cirúrgicas foram realizadas com incisões horizontais únicas sobre as pregas cervicais superior. Resultados: Todas as abordagens cervicais tiveram boas exposições sem dificuldades técnicas para as cirurgias programadas. Não houve complicação. Os pacientes seguiram sem recidiva loco-regional da doença. Conclusões: Não houve dificuldades técnicas ou complicações. Os pacientes tiveram excelentes resultados funcionais e estéticos.
Introduction: The neck incision usually was a concern by several authors regarding the functional aspects. The patients by their turn claim for aesthetic scar. This matter is very important due to the psychological aspects. The best incision in the head and neck cervical skin is horizontal. It contemplates important aspects as the anatomical, good exposition of the organ and all lymph nodal levels in the neck and it respects the natural tension skin fibers. Objective: To observe the single horizontal incisions, fitted inside the superior cervical crease (SCC). It was done to treat neck metastasis of the head and neck malignant tumor. The technical difficulty, complications, function and the aesthetic aspects were observed. Methods: Twenty patients submitted to a single horizontal cervical incision in the SCC for radical neck dissection (6 patients), modified radical neck dissection (4) and supra omohioid lymph node clearance (10) were studied. Results: All procedures had good expositions of the anatomical structures, without technical difficulty for the programmed treatment. There was no complication. Conclusions: There was no technical difficulty or complications. All patients presented excellent functional and aesthetic results.
